Athletic Information

Dear Athlete,

Welcome to Athletics at MATC.  We are glad to have you as part of our team.  If you have any questions regarding our athletic programs, please feel free to contact me at 414-297-7872 or grulkec@matc.edu.

Attached is our athletic eligibility form.  Please complete it and return it to the Athletic Department immediately for processing.  Please keep the following in mind:

Physicals- All student athletes MUST have a physician's clearance before they may practice or compete in athletics.  Athletes have two choices to complete this: 

     You may go to your own physician; there is no form they need to complete, but on the physician's stationary they must indicate that you are cleared for athletic participation.  This document can then be faxed to the Athletic Department at 414-297-6657, or given to your coach.

     OR

     Bring your completed eligibility form to the Athletic Office to obtain authorization for a free MATC-sponsored athletic physical.   When you receive the authorization form you may call our contracted clinics for an appointment:

You must bring a copy of the authorization form to the physical. 
Upon completion of your physical please bring a copy to the Athletic Office.

Transcripts- If you attended any other college before coming to MATC, we will need an official copy of your transcripts to determine your athletic eligibility.  You may have them faxed to the Athletic Department at 414-297-6657.  If you turned in official transcripts at the Mequon, Oak Creek, or West Allis campuses, please ask for a copy at the Admissions Office at your campus.  If you submitted them to the Milwaukee Campus, please indicate that on your eligibility form.
